Platinum Award for VHA's Polytrauma System of CareThe Veteran Health Administration (VHA) Polytrauma System of Care (PSC) has been recognized by Dorland Health's 'Case In Point Platinum Awards' as one of the most successful and innovative case management programs working to improve healthcare across the care continuum.

The Case In Point Platinum Awards recognize both individual professionals and organizations of all sizes in a range of categories that highlight the full spectrum of case management programs, as well as individuals working on the frontlines of managing care – from best acute care case manager to best embedded case management program with dozens in between.

Beginning in 2005, VHA developed the PSC to provide world-class rehabilitation services, and ensures Veterans and Servicemembers with polytrauma transition seamlessly between Department of Defense and VHA, and back to their home communities. This integrated network of over 100 programs across four echelons of care specializes in clinical rehabilitation services, including: assessments and treatments by interdisciplinary teams of rehabilitation specialists; specialty care management; patient and family education and training; psychosocial support; and advanced rehabilitation and prosthetic technologies. 

VHA’s PSC balances access and expertise to provide specialized polytrauma and TBI care at the location closest to the Veteran’s home with the expertise necessary to manage his/her rehabilitation, medical, and psychosocial needs. Polytrauma case managers ensure seamless coordinated care for Veterans Servicemembers in need of TBI/Polytrauma services.

Such services include, but are not limited to:

  • Specialized inpatient and outpatient rehabilitation
  • Emerging consciousness program
  • Transitional rehabilitation
  • Assistive technology labs
  • Polytrauma telerehabilitation
  • Vocational services
  • Community re-entry programs
